Release of Information Consent
PLEASE NOTE: If your child is named in a SAPCR-court order, custody agreement, or other legal document regarding consent for treatment of a minor, I must have a copy of the most current order before I begin services. Request Your Health Care Records
You have the right to request your health care records from us. Please ask for a Consent to Release Information Form which must be completed and submitted to obtain records. Records will be released within the mandated timeframe set by the State of Texas after the signed release is received.

File a Consumer Complaint
To file a complaint, please submit your concerns to the therapist. Should the outcome not be satisfactory, you have the right to contact the Office of the Attorney General at: https://www.texasattorneygeneral.gov/consumer-protection
